Transcripts
Transcript requests may be made in person by filling out a Transcript Request Form available in the Registrar's Office or by mailing or faxing a letter to the attention of the Registrar. Official Transcripts cannot be issued upon demand and must go through Business Office approval.
Mail Requests to: Office of the Registrar, One Macklem Dr., Wilmore, KY 40390 (Without your legal signature your request cannot be processed.)
Fax Requests to: (859) 858-3921 (Without your legal signature your request cannot be processed.)
E-mail Requests to: registrar@asbury.edu (Without your legal signature your request cannot be processed. Please attach a graphic file [.gif or .jpeg] containing your legal signature.)

ALL TRANSCRIPT REQUESTS MUST BE MADE IN WRITING TO INCLUDE THE FOLLOWING:
- Full Name - (Include Middle & Maiden Name)
- Social Security Number - (Necessary for verification purposes)
- Date Of Birth - (Necessary for verification purposes)
- Last Enrolled/Graduation Date At Asbury
- Legal Signature - (If name is printed or typed, your request cannot be honored)
- Complete Address For Transcript Destination (Please be specific as possible)
- Your Current Mailing Address & Phone Number (In case we need to contact you)
- To print a copy of the request form, click on REQUEST FORM. Print the form, complete it, and mail or fax to the address above.
NOTES:
- There is no charge for transcripts but there is a limit of 5 transcripts per request.
- Financial obligations to Asbury must be paid before official transcripts are released.
- Telephone requests will not be accepted.
- Transcripts will not be faxed due to confidentiality and legal restraints.
- Please specify if you would like confirmation of your transcript being mailed.
- Please allow 5-10 working days for processing your request. The Registrar's Office is closed during the week of Christmas.
- This college does not release transcripts or copies of transcripts from other institutions.
