If you are a new enrollee or want to continue participating in the flexible spending accounts click on the link below for instructions:(Direct Deposit is required for reimbursement-have your checking account number and routing number available when enrolling) You will also need your employee key/ID number which can be found on your pay stub in the upper left hand corner or on the confidential personalized benefit letter that was sent to employees through CPO.

Insurance premiums are automatically deducted pre-tax. If you do not wish to have your premiums pre-tax you must complete the following form each year Flexible Spending Premium Waiver Form.
