To open forms, simply double-click the file name below. The form template will open in Microsoft Word. Complete the information requested by tabbing from field to field or by clicking the mouse on the desired field. Once you have completed the form, save it to your hard drive under a new name. You may then print the completed form and/or e-mail the form to your professor, area coordinator, and/or to Mrs. Montgomery in the Fine Arts Office by attaching your newly created file to an e-mail message.
