Medical Records Release
It is the policy of Asbury College Student Health Services to maintain the medical files of full-time enrolled students for seven years either after graduation or last patient contact. After that time all medical records are shredded, in accordance with HIPAA regulations.
To request your medical records, please download our Fax cover page and Patient Authorization. You will need to carefully complete both forms, making sure to sign the Patient Authorization and either mail, fax, or email to:
